Alameda House is a 15-bed transitional living facility that provides rehabilitative skills training and outpatient substance abuse treatment to adults with a priority population diagnosis of Schizophrenia, Schizoaffective, or Bipolar disorder and/or co-occurring substance use issues. Length of stay is 90 days.
Who is Eligible?
To qualify, a person must already be receiving services – working with a case manager / rehab specialist and receiving Level of Care (LOC) 3 or 4 services from Integral Care. Medicaid or other insurance is required. The facility is not able to serve persons who are registered sex offenders.
Referrals
- We are unable to take referrals from the community.If you are a current consumer of Integral Care, please contact your primary worker who can make a referral on your behalf if you are receiving LOC 3 or LOC 4 services. LOC 3 or LOC 4 services means that a worker provides services to you in the community or at your home. If you only see your worker and psychiatrist at the office, it most likely means that you do not qualify; ask your worker to check with Level of Care you are currently receiving.
