E-Merge is an integrated behavioral health collaborative service between CommUnityCare (Federally Qualified Health Clinics) and ATCIC, integrating behavioral health care services at primary health care clinics in an effort to comprehensively address the patient needs. The goal of these services is to assist patients in achieving both improved physical health and behavioral health care functioning by concurrently addressing their expressed needs.
E-Merge is able to provide psychiatric consultation services as determined by the patient’s presenting needs. Behavioral health clinicians (BHC’s) use a team approach in conjunction with the primary care providers to improve the quality of patient care.
Objectives of E-Merge Services are to assist individuals in receiving speciality behavioral healthcare and/or substance use treatment services when indicated. Additionally, E-Merge collaborates with other professional staff in clinics to provide health management and promotion groups for individuals receiving care.

What to Expect
- When a primary health physician or clinical nurse determines the need for a mental health assessment, the patient is referred to E-Merge staff. Referral responses are usually immediate.
- Referrals begin with a 30-minute mental health assessment, which includes both a clinical interview and a Personal Health Questionnaire.
- The questionnaire includes questions about mood, sleeping, anxiety and substance use history used to determine the best plan of care with the patient.
- Typically, the BHCs will see patients for 4-6 brief follow up counseling sessions, although more can be scheduled, if necessary.
