Of the Austin Police Department and Travis County Sheriff’s Department providing nursing assessments, interventions, brief follow-up and service linkage to adults and children/youth in non-clinical community settings. Experience: This position is located in the C.A.R.E. unit of the Behavioral Health Division of Austin Travis County MHMR. Its purpose is to provide community and street outreach, education, HIV testing and limited case management and service coordination work for adults who are at high risk for HIV, Hepatitis C or other sexually transmitted diseases (STDs). Applicant must have experience working in direct care with the target population or with people with mental illness or substance abuse. Must have knowledge of community resources and the ability to access client’s needs and to coordinate client services. Must process the ability to communicate effectively with diverse cultures and socio-economic populations. At least one (1) year experience in case management, HIV/AIDS, and substance abuse required. Spanish speaking helpful.

Experience: The primary purpose of the position is to assess a client’s mental impairments, develop a personalized plan to provide services sought by the client, provide necessary skills training to facilitate the client's success in achieving and maintaining the maximum possible level of independent functioning. Services may include developing skills necessary to obtain/maintain employment or housing, or to obtain services such as medical care, dental care, food, clothing, utilities, financial assistance, transportation, legal assistance, or any other basic human need and the coordination of these services to assist the client in obtaining these services. Must be able to provide Rehabilitative Services to Adults in crisis. Must be able to provide Individual and Group Rehabilitative services in the following three Community Support Services: Symptom Management, and Support Services, Community Living Skills Training and Employment-Related Support and Skills Training. Medication Training and Monitoring utilizing the TIMA Materials and includes, but is not limited to, the sharing of information regarding the purpose and side effects of the medication, overdose precautions, contraindications, and instruction in the self administration of medication. Other included services are instruction in methods to manage stress, strategies and techniques for coping, reality orientation, and other supportive services during times of crisis or episodes of acute psychiatric symptoms. Community Living Skills services focus on improving communitcation skills, interpersonal behaviors, and other skills that will facilitate integration into the community, such as personal hygiene, household tasks, and money management. Training and support in Accessing needed services in the community. Training to improve communication, increase interpersonal interactions and make appropriate leisure time decisions such as voice modulation, body language, eye contact, and avoiding high-risk behaviors. Training to develop natural supports in the community such as social/civic organizations, advocacy groups, religious affiliations, and interpersonal relationships. Must be able to develop individualized Treatment Plans with the Consumer. Must be able to reflect an accurate assessment of the Consumers response to treatment during the course of each service delivery. Must be able to coordinate services with the various treatment teams. Work is performed under the general supervision of the unit manager with moderate latitude for the use of initiative and independent judgment. Experience: The primary function of this position is to provide day-to-day supervision and support for clients residing at the program. A critical component is ensuring the health and safety of the clients and promoting a stable therapeutic environment within the program. The position will function as a member of the treatment team but will be able to work with minimal to moderate ongoing supervision. The individual in this position must have good inter-personal communication skills (able to interact with clients with serious emotional problems in a therapeutic manner). Must be able to be calm and consistent in communicating and enforcing house rules and regulations. The individual in this position must be able to organize meal preparation, cook for clients and supervise clients assisting with this process. The position assigns and oversees clients cleaning their immediate living areas. Should be able to train clients in using laundry facilities and basic household cleaning techniques. Must have the ability to learn the basic stages of change of substance abuse rehabilitation. Must have sufficient writing skills to document in a clear and concise manner.
