The CDM Program is a voluntary program designed to assist individuals with chronic disease conditions and will have two teams: at an upcoming north Austin site and the Dove Springs Center opening in October 2013. Based on biological markers for chronic disease conditions and health risk assessment identified needs, consumers will be referred to various elements of the program corresponding to their chronic disease conditions; smoking cessation, activity, diabetes and obesity. These elements are staffed by professionals in these specialty areas as well as peers who have received specialized training in recovery oriented principles, whole health training, InShape training, health navigator training and smoking cessation training. These peers then work with the enrolled consumers to develop whole health plans and then coach them to success in self managing their conditions.
